We are a web development company. We have a form that submits using PHP to a MySQL database. This is a vehicle accident report form, in which the user submits information about the vehicle accident.
We need an addition to this form that allows the user to illustrate the accident.
To achieve this, we need an item added into this form that allows the user to drag and drop and rotate icons for the driver's car, other vehicles, traffic light, tree and an other obstacle (representing anything else not provided as an icon).
see attached the blank diagram that the user will be able to drag and drop these icons on top of.
The resulting information is to be done in a format that we can submit using PHP into the MySQL database and then extract to be viewed by the admin. We will do this part of the work but you must include the code to do this to illustrate what you deliver works.

Display Map Coordinate/radius

As a bonus, I would like to have the center point be a custom graphic that I provide rather than the standard "flag" used in a mapping program, but I'm not sure whether that's possible or how much extra that would be (this is a "nice to have", but not critical).

Contact form in Flash + embedding .swf map in template

Boonex Dolphin Join Form Fix
0.1 and am having a problem with the Join Form under Description. The minimum length is set at 65534 characters (have no idea why). When I try and change it it the admin, it always resets to 65534. No matter what I do it resets to 65534.
I want it to be a required field (can be set in the admin) with a minimum of 10 characters and a max of 10,000.

Aweber Opt-in Form

Hi, my submission form on my website http://www.fastwebsitedesign.net used to work... now I've noticed that even though entries get submitted, they don't make it to my email.

There is also 2 minor minor changes need doing. Such as a drop down box instead of the current radio button group.
Then just some basic SEO... nothing too hectic as the budget for this project is tight and strict.

ls to correct a problem where a website is not sending out a message from the contact form. Must be able to communicate in very good English in writing, by skype or google chat if you are out of the country [USA]. This should not be a big problem for someone with good skills, and if I like working with this person, I may use them again for future work. Please do not respond if you do not meet the qualifications above. Small budget on this one, just a correction to existing code.
